Approach
The goal of the Opti-Q Multimorbidity project was to implement the practice recommendations for multimorbidity in primary care in different regions of Switzerland. The focus was on patient-centered care and on strengthening interprofessional collaboration in various primary care settings. The project was supported, among others, by the PGV Fund of Health Promotion Switzerland Foundation.
Findings
An interprofessional expert group from practice and research developed four practice-oriented application tools that were tested in 13 interprofessional teams in three language regions of Switzerland: a holistic patient assessment, a care passport for patients, a medication checklist, and an overview of self-management resources for multimorbid conditions. The participating teams attended a joint workshop on interprofessionality and tested the application tools in everyday practice.

Two scientific publications are in progress.
Commissioned by the Federal Office of Public Health (FOPH), an additional project was developed outside the Opti-Q framework, utilizing existing synergies: the creation of a resource that also contributes to strengthening the quality of care.
